What is a Flood Certificate? Flood Certificate Example
Flood Certificate Example to help define a Flood Certificate, technically referred to as an Elevation Certificate, is an official document that is used to determine flood risk and, if applicable, flood insurance cost based on that risk.
Following are Flood Certificate sample pages. Each page has a description of the information required on the page and a link to a full size PDF file of the page for a clear view
All of the pages have a reference to the property address at the top and being self-explanatory, will be skipped in the page descriptions
Flood Certificate Example, Page 1
- Section A on Page 1 is for the data of the structure being certified.
- Section A3 - This provides a form of legal description of the property location.
- Section A4 - It is important to note if the structure is a residence or not. It may be a garage or work shed that is not occupied as a residence.
- Section A5 - This provides a more specific location of the structure location by latitude and longitude. This will eliminate any confusion on a property with multiple structures.
- Section A6 - A bit of redundancy here. Page 5 and Page 6 are specifically designed to have the photographs which are required.
- Section A7 - The Building Diagram Number is provided here. The diagrams help to distinguish the different types of structures. Diagram 1 defines a typical residence or retail building. Diagram 2 defines a structure with a basement. Diagrams 3 and 4 define split-level structures. Diagrams 5 and 6 are structures on piers. Diagrams 7, 8 and 9 are different types of structures with a crawlspace.
- Section A8 - This provides specific crawlspace information for applicable structures.
- Section A9 - This provides specific information about an attached garage if it is applicable to the structure.
- Section B on Page 1 provides specific information found on the FEMA map that depicts the location of the flood zones..
- Section B1-B3 - This is information about the municipality, county and state that is found in area shown on the map.
- Section B4-B7 - This is technical information about the map definition and the time-frame of the data depicted on it.
- Section B8-B11 - This is for information about the flood zone the structure is found in, how the flood zone was determined and the specific type of data provided
- Section B12 - This provides information for a structure that is inside a protection barrier, such as a levee.

- Section C on Page 2 is only applicable to the flood zones that have a specific elevation above sea level calculated for the flood.
- Section C1 - Describes the status of the structure being certified. Sometimes the flood insurance risk is being determined for a proposed structure.
- Section C2 - The technical data for benchmark that was used to determine the elevations needs to be listed.
- Section C2 (a, b, c, d, e, f, g and h) These are specific elevations that were measured around the structure as required.
- Section D on Page 2 provides the detailed information about the person certifying with their professional license the accuracy of the data provided on the Flood Certificate, including the professional seal.

- Section E on Page 3 is only applicable to the flood zones A and AO. They are areas prone to flooding and do not have a specific elevation calculated for the flood.
- Section E1-E4 - These are specific elevation differences relative to the higest or lowest grade adjacent to the structure.
- Section E5 - Is rarely applicable. It is only required an AO flood zone where no flood depth was provided, which is unusual here in Maricopa County, Arizona.
- Section F on Page 3 provides the detailed information about the person certifying the accuracy of the data provided on the Flood Certificate.

- Section G on Page 4 is only applicable to the local municipality that manages the flood zones. Sometimes this is used to provide a base flood elevation at a specific location in a flood zone, such as Zone A, which does not have a base flood elevation calculated.

- Pages 5 and 6 of the Flood Certificate provide pictures of the structure, front, back and sides, along with the date the picture was taken.
